As part of our commitment to sustainability, this year’s summer health mailings is available electronically. Grade-specific Health Forms for your child(ren) are now accessible on the Onteora Central School District Health Services Webpage. You can view and print the necessary forms directly from the site. Please send all forms back to the high school's health office.

If you do not have access to a printer and require paper copies, please email Sabrina Blakely, High School Nurse, at sblakely@onteora.k12.ny.us, and copy Lisa Ryan at lryan@onteora.k12.ny.us. They will be happy to mail the required forms to you.
